Synopsis

The challenges resulting from the current demographic and epidemiological contexts pose to health care are addressed. On this basis, an in-depth analysis of primary health care and integrated long-term care reforms is made. From this analysis results proposals of change focused on future challenges and opening horizons beyond Integrated Continuous Care, because the responses to those depend on the entire system and not just from a level of care. Thus, a reflection is developed on the implications of person-centered care, as well as the integration and continuity of care, a model of development to integrated continuous care is proposed. For this model, the multiple possibilities resulting from the new information and communication technologies applied to health are considered. Finally, the implications of a new development model for health services are briefly analyzed, having as referential bullet points the process of change in the health system and the issue of knowledge management, all of them framed in a wellbeing economy.
